Q: Has anyone tried pramipexole for depression and anhedonia not helped by the SSRI’s?
Did it work?

A: No I haven’t, but I am surprised if it has been prescribed for depression/anhedonia – in the UK it is licensed for the treatment of Parkinson’s disease and related disorders (sometimes psychosis).

It has some worrying side effects as it can induce some compulsive behaviours such as gambling, binge eating and hypersexuality..

Best of luck, but I would be tempted to have a further cat with your GP about this.

Q: I have Severe Restless Lehs Syndrome (SRLS) and it is ruining my life.?
I have SRLS, and currently take 5mg of Repreve daily.
It cost me $300 a month, but makes me so depressed I am finding it hard to cope.
Mirapax (Pramipexole) is being released in my country next month, I am swithching straight away.
Do you think this may help, and any other suggestions that may help me?
ps Can you take Madopar with these drugs and would that help?

A: How bad is your SRLS?
You need to weigh your options.
Is being depressed worse or better than having the full effects of RLS?

My suggestion is to switch to Mirapax, and if that doesn’t work, try taking small iron supplements. Iron can help increase ferritin levels, which helps 60% of people with RLS. Taking too much iron, however, can harm you, so you cannot take it as a medicine.
Requip is also a possible medicine choice.

Here’s what I take for depression and anxiety — I’m 35 y/o and had 12 courses of ECT 8 years ago. I don’t do drugs and I drink occasionally (infrequently).

Meds:
Wellbutrin XL 150 mg am
Prozac 20 mg am
Clonazepam 0.5 mg: 3 in am, 2 at noon, 2 at 6p, and 3 at bedtime (total 5mg/day)
Lyrica 75 mg at bedtime
Pramipexole 0.50 mg at bedtime (for SSRI sexual side effects)
Seroquel 75 mg at bedtime (to help sleep)

I want to quit taking so many drugs — women run when I tell them … they’re expensive and I’ve suffered withdrawal from clonazepam twice (*awful* experience from missing 2 days)

How best should I go about weaning off that much clonazepam?

Please answer only if you know. I’m a busy professional with a stressful job with a lot of responsibilities. Only my psychiatrist and I know I’m taking anything.

A: Hey Dave..been there..done that too. Here’s the fundamental problem. Unless your antidepressants are high enough, when you try to go off klonopin, your anxiety/panic will come back. So 20 mg of prozac may very well not be enough to replace the antianxiety that the klonopin addresses. As far as getting off the klonopin, you have to take it real slow, no more than 0.5 mg per day for a month, then another 0.5 mg per month, etc. as you are very sensitive. Some people have to stay on an antianxiety for a long time in addition to their antidepressant. As long as you are stable, just keep searching for the right girl who understands. Hey, half of america is on something or other. Take care of yourself.

A: Hi. I’m not an expert but the person who is in the best position to answer your question, is the doctor who prescribed the medication to you.

He or she knows your medical history and would be able to advise you how whether you can reduce the medication and how to go about it. Tell the doctor your concerns about being on so many medications and see what he says.

I know I haven’t answered your question directly but frankly topics such as reducing medication should only be discussed with your own doctor.

A: There are ways to improve your symptoms of restless legs syndrome at home.

Try:

Exercise. Regular, moderate exercise may reduce symptoms. Avoiding long periods between activity or sudden bursts of intense activity may also be helpful. Talk to your doctor before starting an exercise program.
Heat or cold. Your symptoms may be relieved by bathing in very hot or very cold water. Applying a heating pad or ice bag to your legs may also help symptoms.
Changing your sleep schedule. Fatigue can make your symptoms worse. Because symptoms typically improve around 4 a.m. to 6 a.m., try going to bed later than usual or allowing extra time for sleeping in to help you get the rest you need.
Stretching and massage. You may be able to control your symptoms by gently stretching and massaging your legs before bed or as discomfort begins.
Avoid:

Caffeine and alcohol. These may make your symptoms worse.
Certain drugs. It may help to avoid antidepressants, antihistamines, cold and sinus medicine, antipsychotic medicine, and certain antinausea drugs (metoclopramide or prochlorperazine). Taking products that contain these ingredients may make your symptoms worse. Your doctor needs to know what medicines you are taking in order to rule them out as a cause of your symptoms.
Being confined for long periods. Try to plan for situations where you will need to remain seated for long stretches. For example, if you are traveling by car, plan to make occasional stops so you can get out and walk around.
Excessive exercise. Although moderate exercise may help relieve symptoms, unusually intense workouts may make them worse. Try to identify at what level exercise helps and at what point it triggers restless legs syndrome
